Tarsal tunnel syndrome (TTS) can cause tingling, swelling, numbness, or sharp pain along the inside of the ankle and into the sole of the foot. The discomfort may start subtly but can become increasingly debilitating if left untreated.
What Is Tarsal Tunnel Syndrome?
Tarsal tunnel syndrome occurs when the tibial nerve, which runs through the tarsal tunnel near the ankle, becomes compressed. This narrowing can result from prolonged or repeated pressure on the tunnel, leading to nerve irritation and inflammation.
Over time, the symptoms can progress, affecting your ability to walk, stand, or stay active without pain.
Common Causes of Tarsal Tunnel Syndrome
Tarsal tunnel syndrome may develop from a combination of external factors and anatomical or medical conditions.
External Causes
- Wearing tight or poorly fitting footwear
- Injuries such as ankle sprains, fractures, or dislocations
- Post-surgical scarring around the ankle
- Conditions like diabetes or inflammatory arthritis
Internal or Natural Causes
- Swollen or enlarged veins
- Inflammation of the tendons (tenosynovitis)
- Tendon degeneration (tendinopathy)
- Scar tissue formation around the nerve (perineural fibrosis)
Additionally, individuals with flat feet or higher body weight may be more prone to developing TTS due to increased pressure on the tibial nerve.
Why Early Diagnosis Matters
Without timely evaluation and treatment by a foot and ankle specialist, tarsal tunnel syndrome can lead to permanent nerve damage. In severe cases, this may interfere with your mobility and make everyday activities painful or impossible without assistive devices or support.
How Physical Therapy Can Help
While some cases of TTS may require surgical intervention, physical therapy is often an effective and less invasive treatment approach. In fact, surgery for tarsal tunnel syndrome carries a potential complication rate of around 13%, according to the National Center for Advancing Translational Sciences.
Physical therapy focuses on relieving symptoms, restoring function, and preventing further damage. It’s a personalized approach that can help patients remain active and avoid surgery when possible.
Common Physical Therapy Treatments for TTS
A physical therapist may recommend a tailored plan that includes:
- Muscle-strengthening exercises for the lower leg to support foot mechanics
- Balance and coordination exercises to address any functional disruptions caused by nerve compression
- Nerve gliding techniques to promote healthy nerve movement and reduce irritation
- Orthotic guidance to help position the foot correctly and reduce pressure on the tibial nerve
Ongoing Education and Support
Education is also a key part of physical therapy. Your therapist will help you recognize the signs of worsening TTS and teach strategies to manage symptoms. With greater awareness, patients are often able to prevent flare-ups and improve long-term outcomes.
What to Expect from Physical Therapy
With consistent treatment—often for a minimum of four weeks—many individuals experience noticeable relief from tarsal tunnel syndrome. Continued therapy may yield even better results and help avoid the need for surgery altogether.
