A conversation with Dr. David Dare, MD
Shoulder arthritis can significantly impact your daily life, limiting mobility and causing persistent pain. While shoulder replacement surgery is often the most effective long-term solution, many patients ask about non-surgical alternatives. In this Q&A, Dr. David Dare, a board-certified orthopedic surgeon at Raleigh Orthopaedic Clinic, shares insight into current treatment options for shoulder arthritis, including both conventional and emerging therapies.
Q&A with Dr. David Dare
Finding the Right Arthritis Treatment for the Shoulder
Q: I have very severe shoulder arthritis. What are my options?
A: We are constantly in search of new treatments for shoulder arthritis.
Arthritis is essentially the degeneration of the cartilage—the smooth surface—on the ball and socket of the shoulder joint. Once this cartilage breaks down, patients typically experience pain, stiffness, and decreased function.
While shoulder replacement surgery is a well-established and highly successful treatment for severe shoulder arthritis, many patients understandably ask about non-surgical alternatives.
One such option is the off-label use of hyaluronic acid injections, which are FDA-approved for knee arthritis but not officially approved for the shoulder. Because of this, patients must pay for the medication out of pocket. The injections are typically administered in the office over the course of three to five weeks and may help reduce inflammation and improve joint lubrication.
Cortisone injections are another frequently used option. These can provide effective, short-term relief for shoulder arthritis by reducing inflammation in the joint.
As for platelet-rich plasma (PRP) therapy, while it has shown promise in treating soft-tissue injuries such as tendonitis, current research does not support its effectiveness in regenerating or healing cartilage in arthritic joints. Therefore, its use in cases of advanced shoulder arthritis remains limited.
Ultimately, the best treatment plan depends on the individual patient’s condition, goals, and response to previous therapies.
