Request for Medical Record Copies
It is required by law to obtain your permission before releasing any copies of your medical records. Raleigh Orthopaedic utilizes HealthMark Group to provide medical records in a safe, secure, and timely manner.
Complete your medical record request entirely online through HealthMark Group (details below). There is no charge for copies of medical records, whether requested by the patient or sent directly to other healthcare providers. For answers to your questions or to ask about the status of your request, please call HealthMark Group at 800-659-4035 or email status@healthmark-group.com.
Why Use HealthMark Group
HealthMark Group provides you with real-time online status, tracking, and secure messaging. Once records are ready, you will receive an email notification prompting you to log into your HealthMark Group account and download an electronic copy of your requested medical records. The entire process is completed online, without you having to leave your desk.
