Dear Patient,

After practicing as a physiatrist for 25 years, and for Raleigh Orthopaedic Clinic for the past 18 years, it seems it is an appropriate time to retire.  I have thoroughly enjoyed caring for and treating patients during this rewarding time.  My official date of retirement from Raleigh Orthopaedic Clinic (ROC) will be July 26, 2018.  After July 26, 2018, the appointment schedulers at ROC will be available to assist you in obtaining follow-up care with another ROC physician or to make appropriate referrals elsewhere if necessary.

I heartily recommend Raleigh Orthopaedic Clinic for your follow-up care.  I am confident that ROC can satisfy all your needs and provide the same high quality of care that you have received from me.  Raleigh Orthopaedic Clinic remains one of the largest and premier orthopedic groups in the Triangle and surrounding regions of central North Carolina, with almost 100 years of being committed to providing high quality orthopedic care that involves a whole body approach.  The physicians are board certified and fellowship-trained in their specialties and have the necessary credentials and expertise to manage your care.

However, if you choose to relocate your care, ROC will assist you in any way that they can in order for there not to be an interruption in your care.  Because your confidentiality is one of the utmost important aspects of ROC’s philosophy, it will be necessary for you to complete the attached “Authorization for Release of Medical Information” form which will provide them your authorization to release your medical records.  Without this form they cannot release your records.
